Dear Parent or Guardian,

Please completely fill out the necessary information below.
Help us by making sure your student(s) understand our Behavioral Expectations, Disciplinary Action Steps, and the Van Rules as presented here. These standards apply from the moment your student enters our care; from the van ride to and from church as well as all our services and special events both on and off campus.
Please read, explain, fill out, and sign the Behavior/Disciplinary form with your student. If this form is not filled out, your student will not be allowed to ride the church van or be permitted to attend our Student Ministry services or special events.
For security purposes, parents of elementary students who are dropped off by a parent or guardian, must check their student(s) in and out of services. Upon check in you will receive a tag that corresponds with your student, and the tag must be presented upon picking your student up from service.

Our goal is not to limit or exclude any student, but rather to include every student as we help guide them in their relationship with God through Jesus Christ and teach them to become men and women of integrity and grow into the gifts and callings God has placed in their lives.
